Warren Park Hockey League reserves the right to accept or reject any Application/Registration Form and acceptance is determined at the sole and absolute discretion of the officers of Warren Park Hockey League and may not be disputed. Any player or parent/guardian not conducting themselves in sportsmanlike manor, or disrespecting or abusing directors, officers, coaches, managers, officials, volunteers, other players or parents/guardians are subject to disciplinary action, including suspension or ejection from the league. If disciplinary action is required, the penalty will be determined at the sole and absolute discretion of the officers of Warren Park Hockey League and may not be disputed. IF SUSPENSION OR EJECTION IS DEEMED FITTING, ENTRY FEES ARE NOT REFUNDABLE. The placement of players will be determined at the sole and absolute discretion of the officers of Warren Park Hockey League and may not be disputed. Placement of players will be based on age, skill level and availability. Unless two siblings are close enough in age and skill level, we cannot accept requests to place players on certain teams or with certain coaches. In order to balance the teams, players may be required to move to a different team or division after the start of the season. Players registered in the Warren Park Hockey League my not play in any other registered league unless a release form is granted and vice versa. By filling out this application, you agree to release and hold harmless Warren Park Hockey League, their legal representatives, advertisers, sponsors and each of their respective parents, subsidiaries, affiliates, directors, officers, employees, volunteers and agents from any and all liability for any injuries, loss or damage of any kind to the player, and his or her parent/guardian arising from or in connection with participation in this league, whether or not such damages are foreseeable. All players participating in the Warren Park Hockey League are covered by accident insurance. In order to ensure coverage, proper equipment, including CSA approved helmets and CSA approved neck guards, must be worn by all players at all times. There is no body checking allowed in Warren Park House League hockey.
